29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Machine Shop - On or about March 11, 2020 and at times prior to, the lower non-working portion of the blade on a MEP bandsaw was not equipped with a guard, thus exposing employees to caught-in hazards. b) Fab Shop - On or about March 11, 2020 and at times prior to, the lower non-working portion of the blade on a MEP bandsaw was not equipped with a guard, thus exposing employees to caught-in hazards. c) Machine Shop - On or about and at times prior to March 11, 2020, chuck on a European Lion lathe machine was not guarded, thus exposing employees to a struck-by hazard. d) Machine Shop - On or about and at times prior to March 11, 2020, the chuck on a Dynamic lathe machine was not guarded, thus exposing employees to a struck-by hazard. e) Machine Shop - On or about and at times prior to March 11, 2020, the chuck on Jet lathe machine was not guarded, thus exposing employees to a struck-by hazard. f) Machine Shop - On or about and at times prior to March 11, 2020, the chuck on the big Summit lathe machine was not guarded, thus exposing employees to a struck-by hazard. g) Machine Shop - On or about and at times prior to March 11, 2020, the chuck on the Summit lathe machine was not guarded, thus exposing employees to a struck-by hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Laser Shop - On or about March 11, 2020, the outer sheathing on the flexible cord (extension cord) used to energize a Dayton pedestal fan was pulled away from its connection point on the male end of the cord, thus exposing employees to electrical hazards.
