29 CFR 1910.26(c):  Portable dockboards are secured by anchoring them in place or using equipment or devices that prevent the dockboard from moving out of a safe position.  When the employer demonstrates that securing the dockboard is not feasible, the employer must ensure there is sufficient contact between the dockboard and the surface to prevent the dockboard from moving out of a safe position;    a) On or about 3/17/2020 the employer did not ensure that the portable dockboards being utilized to load/unload laundry carts (200-600 lbs.) were secured to the trucks being loaded or unloaded, as a result this exposed employees to a tripping hazard and crushing hazard.     b) On 2/28/2020 an employee was pulling a laundry cart (300-400 lbs.) out of the back of a truck, as the wheels came in contact with the edge of the ramp the force caused the ramp to slide, this resulted in a gap between the truck bed and the dock, this allowed the employee's foot to fall in between the two just as the cart fell into the gap and impacting the employee's right foot.

29 CFR 1904.29(b)(3): How quickly must each injury or illness be recorded? You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    a) On or about 3/17/2020 the employer did not ensure that a recordable injury that occurred on 2/28/2020 was recorded on the OSHA 300 Log and a 301 Incident Report.  An employee sustained a fracture, this resulted in the employee missing three days of work.
