29 CFR 1917.48(a)(1): Danger zones at or adjacent to conveyors were not guarded to protect employees. (a) Kloosterboer Warehouse - On or about March 10, 2020 and at times prior thereto, employees were exposed to being caught in the pinch and nip points of the conveyor system in the Kloosterboer warehouse while offloading the processed fish boxes from FT Northern Yaeger. In the event and employee were to get caught in the pinch and nip points of an unprotected conveyor system is likely to cause severe lacerations and/or amputation.

29 CFR 1918.24(b): Fixed ladder(s) were visibly (or known to be) unsafe and the employer did not identify such ladders and prohibit their use. (a) FT Northern Yaeger - Freezer Hold Ladder - On or about March 10, 2020 and at times prior thereto, employees were exposed to a slip, trip and fall in that the ladder used to access the freezer hold had uneven spacing between the second and third rungs. The spacing between the rungs was 11" whereas the spacing between the second and third rung was 16".
