29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): undefined :    a)  1 Vanderbilt Blvd., Florham Park, NJ.  Masons performing bricklaying operations were exposed to falls of 29 feet through at least an 18 inch gap to the ground below as 2 platform planks were removed by scaffold dismantlers and workers on that level were exposed to falls as the platform was not fully planked or decked                                          Violation occurred on or about 03/18/2020.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   a)  1 Vanderbilt Blvd., Florham Park, NJ.  Masons performing bricklaying operations were exposed to falls of up to 29 feet to the ground below as masons were using the scaffold end frames for access where the spacing between rungs in the frames was 19 inches and not designed for climbing.                                          Violation occurred on or about 03/18/2020.

29 CFR 1926.451(f)(7):   Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; such activities were not performed only by experienced and trained employees selected for such work by the competent person:  a)  1 Vanderbilt Blvd., Florham Park, NJ.  Masons performing bricklaying operations were exposed to falls of up to 29 feet to the ground below as the scaffold erection and dismantling team had not been trained nor were under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration to ensure other masons were not exposed to falls when scaffold components, such as but not limited to scaffold planks, were dismantled in advancing the scaffold upward.                                          Violation occurred on or about 03/18/2020.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a)  1 Vanderbilt Blvd., at the Interior of the Scaffold's 4th Level, by the Elevator Shaft, Florham Park, NJ.  Masons performing bricklaying operations were exposed to falls of 29 feet to the ground below as when platform planks were removed, the open side of a platform, not the side next the wall being laid, was not guarded with guardrails nor were personal fall arrest systems used.  b)  1 Vanderbilt Blvd., at the Exterior of the Scaffold's 5th Level, Where the Rough Terrain Forklift Truck Loaded Blocks, Florham Park, NJ.  Masons involved in bricklaying operations were exposed to falls of 29 feet to the ground below when offloading blocks and concrete buckets from the forks of the rough terrain forklift as scaffold guardrails were down leaving a 7 to 9 feet opening to fall through and no personal fall arrest systems were in use.                                   Violation occurred on or about 03/18/2020.

29 CFR 1926.454(a):   The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a)  1 Vanderbilt Blvd., Florham Park, NJ and at the Corporate Offices at 1248 Durant Street, Elizabeth, NJ.  Masons performing bricklaying operations were exposed to falls from the fabricated frame scaffolding as the employer had not provided instruction or training on scaffold hazard recognition and how to control or minimize these hazards.                                          Violation occurred on or about 03/18/2020.
