29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. On or about March 24, 2020, at the above addressed jobsite, an employee conducting roofing operations, twenty-two (22) feet above the ground, was not protected from falling. Byrne-Johnson Roofing, Inc. was previously cited for a violation of an equivalent occupational safety and health standard, 1926.501(b)(13), which was contained in OSHA inspection number 1264718, citation number 1, item number 1 and was affirmed as a final order on November 14, 2017, with respect to a workplace located at 7575 W. 175th Street Tinley Park, IL 60477.

On or about March 24, 2020, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
