Danny Miller dba D&A Roofing, 201 West 26th Street, Erie, PA  16508, on or about March 25, 2020 and times prior thereto:  Employees were utilizing a Werner metal ladder to access/egress the roof of the structure.  This metal ladder was being used as part of a ladder jack scaffolding system.  The metal ladder jack scaffold plank/ladder was in contact with the residential electrical drop service.  In addition, employees were utilizing electrical equipment including, but not limited to, flexible cords and an air compressor.  Employees did not have training on safe electrical work practices.  B.)  Danny Miller dba D&A Roofing, 201 West 26th Street, Erie, PA  16508, on or about March 25, 2020 and times prior thereto:  Employees were utilizing pneumatic tools to affix roofing materials to the roof of the structure.  Employees did not have training on safety requirements associated with the use of pneumatic tools.
