29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) CJ Restoration at 4540 N Argonne Street, Denver, CO 80249: On April 14, 2020 the controlling employer did not ensure that employees were protected from injury, in that four out of five employees installing shingles on a two-story residential home, with a roof pitch of approximately 4-in-12, were not utilizing a fall protection system. This condition exposed four out of five employees to a fall hazard of approximately 15 feet.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) CJ Restoration at 4540 N Argonne Street, Denver, CO 80249: On April 14, 2020 the site supervisor for CJ Restoration was not wearing a hard hat while working from the ground to direct subcontractor roofers. This condition exposed one employee to overhead hazards.
