29 CFR 1904.4(a): The employer required by this Part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury and illness that met the recording requirements in 1904.7:  The employer failed to accurately record illnesses on the OSHA Form 300 or equivalent for the calendar year 2020.  At a worksite located at 200 N Banquet, Marshall, MO:  Employees were exposed to COVID-19.  On September 11, 2020, US DOL-OSHA confirmed that the employer did not record the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2020.      a) On or about April 13, 2020, an employee who worked second shift dinner line tested positive for COVID-19.  This employee was hospitalized on or about April 16, 2020, until discharge on or about May 3, 2020.  The employee's expected return to work date was July 20, 2020. The employer failed to record the days away from work.      c) On or about April 15, 2020, an employee who worked second shift pie line tested positive for COVID-19. This employee was hospitalized on or about April 27, 2020, until discharge on or about June 2, 2020.  The employee's expected return to work date was July 20, 2020.  The employer failed to record the days away from work.
