29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  244 Worcester St, North Grafton, MA:  On May 18, 2020 and for many months prior, the employer had not performed the annual periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  In 2019, three machine procedures were inspected using the Annual Review log form and none of the multi-energy source furnaces had been inspected. Additionally, the employer did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under each energy control procedure they were authorized to use.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn and struck by hazards when they bypassed the safety devices of the multi-burner furnaces.  244 Worcester St, North Grafton, MA:  On May 18, 2020 and at times prior, employees bypassed safety devices when lighting furnace 662.  On May 18, 2020, three employees were injured when the gas inside of furnace 662 ignited during lighting with safety devices bypassed.
