29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: On or about May 20, 2020, on the loading end of SMETCO #2, employees were exposed to electrical hazards while working near an electrical junction box that had conductor wires entering the box without the box being effectively closed.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 20, 2020, and at times prior to: b. In the pallet stacking area of SMETCO #1 pallet sorting machine, employees were exposed to caught-by hazards while working near rotating and other moving parts of the pallet stacker without a guard to protect employees from the rotating parts or nip points of the stacker. c. On the back side of SMETCO #1 pallet sorting machine, employees were exposed to caught-by hazards while working near rotating parts of the pallet conveyor that had a physical guard that was broken without repairing the guard to protect employees from the rotating parts. d. In the pallet stacking area of SMETCO #2 pallet sorting machine, employees were exposed to caught-by hazards while working near rotating and other moving parts of the pallet stacker without a guard to protect employees from the rotating parts or nip points of the stacker. Greatwide Distribution Logistics, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1365408, citation number 1 item number 1 and was affirmed as a final order on February 11, 2019, with respect to a workplace located at 225 Portwall Suite 300, Houston, TX 77029. Greatwide Distribution Logistics, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1401690, citation number 2 item number 1 and was affirmed as a final order on June 28, 2019, with respect to a workplace located at 225 Portwall Suite 300, Houston, TX 77029.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about May 20, 2020, in the pallet painting area, employees were using a portable pedestal fan with a cord that was not visually inspected before each shift and presented damaged to the outer insulation of the cord.
