29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Manuel Aguilar locate at 560 Weaver Ridge Drive, Marysville, OH 43040:  On and preceding May 21, 2020, employees were performing roof work on a two story residential home at heights up to 25 feet without fall protection or other PPE. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Manuel Aguilar locate at 560 Weaver Ridge Drive, Marysville, OH 43040:  On and preceding May 21, 2020, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were installing shingles and other roofing materials on the second floor at heights up to 25 feet. The employees were not utilizing proper eye or fall protection which exposed them to falls up to 25 feet and struck by hazards to the eyes.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Manuel Aguilar locate at 560 Weaver Ridge Drive, Marysville, OH 43040:  On and preceding May 21, 2020, the employer did not ensure that employees used appropriate eye or face protection when exposed to eye or face hazards while using pneumatic nail guns. Roofers were installing shingles and other roofing materials on the second floor using pneumatic nail guns powered at 120 PSI. The employees were not utilizing proper eye protection which exposed them to struck by hazards to the eyes.

Roofers were installing shingles and other roofing materials on the second floor at heights up to 25 feet without fall protection. The following condition exposed them to falls up to 25 feet.
