29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a guardrail system, safety net system, travel restraint system or personal fall arrest system:    Employees were exposed to fall hazards at the Grainfield, KS location. Employees engaged in auger removal activities on the scale house were exposed to a fall hazard of approximately 9 feet from the walking-working surface of the upper landing surface to the ground. Employees were exposed in that a fall protection system was not in place.

Employees were exposed to struck-by hazards at the Grainfield, KS location. Employees engaged in material handling activities using the Caterpillar Telehandler, Serial #TBN00750 were exposed to struck-by and caught in-between hazards in that comprehensive forklift training was not provided prior to permitting forklift operations.

Employees engaged in Feed Mill operations while using the Ferrell-Ross Material Processing machine, serial # 7452, located within the feed mill were exposed to caught-in hazards in that the two rear 34 inch long shafts were not guarded.

In the engineering room, electrical equipment was installed in an unsafe manner, exposing employees to an electrical shock and fire hazards in that two 110VAC electrical outlets were not secured to the wall, additionally, electric wires on the same circuit were not installed within conduit, exposing wires to damage.

Employees engaged in electrical panel or activities that included the interior of the closet located in the south scale building were exposed to shock hazards in that the electrical panel located in the west hallway closet was missing the enclosure panel.

A work related incident which an employee was hospitalized was not reported to OSHA within 24 hrs at the Grainfield, KS location. The incident occurred on 05/22/20 at around 1545 and reported to OSHA on 05/26/20 at around 1530.
