29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems    (a) Segura Roofing, LLC., at 2078 S. Pontiac Way, Denver, CO 80224: On or before May 19, 2020, the exposing employer did not ensure that employees were protected from injury, in that approximately 25 employees were removing and affixing roof shingles on 5 different commercial storage building roofs with a 6/12 pitch: without wearing fall protection.  This condition exposed employees to fall from height hazards of 8 feet, 4 inches to 16 feet to the asphalt below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Segura Roofing, LLC., at 2078 S. Pontiac Way, Denver, CO 80224: On or before May 19, 2020, the exposing employer did not ensure that each employee engaged in commercial roofing operations were trained to recognize the hazards of working at elevation and trained to use fall protection.  Employees were engaged in roofing activities, working near the edge of a single story, 6/12 pitch, commercial roof without the use of fall protection systems. This condition exposed employees to a fall of 8 feet, 4 inches to 16 feet to the asphalt below.

Pontiac Way, Denver, CO 80224: On or before May 19, 2020, the exposing employer did not ensure that anchorages used for attachment of personal fall arrest equipment was capable of supporting at least 5,000 pounds per employee attached, and was installed under the supervision of a qualified person.  Four employees used the same anchorage point when installing shingles on a commercial roof.  This condition exposed employees to a fall hazard of 8 feet, 4 inches to 16 feet to the asphalt below.

Pontiac Way, Denver, CO 80224: On or before May 19, 2020, the exposing employer did not ensure that each employee exposed to fall hazards was trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used.  Four employees were not trained on the correct use of personal fall arrest systems in that all used the same anchorage point when installing shingles on a commercial roof.  This condition exposed employees to a fall hazard of 8 feet, 4 inches to 16 feet to the asphalt below.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    (a) Segura Roofing, LLC., at 2078 S. Pontiac Way, Denver, CO 80224: On or before May 19, 2020, the exposing employer did not ensure that employees who were using pneumatic nails guns to affix roof shingles were wearing eye or face protection. This condition exposed employees to the hazard of projectiles to the eyes.     Segura Roofing, LLC.,was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1373391, citation number 1, item number 1 and was affirmed as a final order on March 18, 2019, with respect to a workplace located at 955 Royal Crown Lane, Colorado Springs, CO 80906
