29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On and before August 22, 2023, the employer did not ensure that their employees were protected from fall hazards while conducting roofing activities in that they did not ensure that the 12-foot metal extension ladder was secured and extended 36 inches above the landing surface. This condition exposed employees to fall hazards in excess of 6 feet.

This condition exposed workers to a fall hazard. Segura Roofing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1583172, citation number 1, item number 1, and was affirmed as a final order on March 25, 2022, with respect to a workplace located at 5172 S Julian St, Littleton, CO 80123.
