Aaron's Roofing & Construction, LLC, 2802 Lafayette Road, Erie, PA  16506, on or about May 27, 2020 and times prior thereto:  Employees were performing roofing activities including, but not limited to, removing shingles from the roof of a wood-frame single family dwelling.  Employees were exposed to a fall hazard of 10.75-feet and were not utilizing means or methods of fall protection.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED:  Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder:    A.)  Aaron's Roofing & Construction, LLC, 2802 Lafayette Road, Erie, PA  16506, front / east side of the structure, on or about May 27, 2020 and times prior thereto:  Employees were using a ladder to access/egress the roof of the structure.  The ladder was only extended 2-feet above the upper level landing surface (roof).    B.)  Aaron's Roofing & Construction, LLC, 2802 Lafayette Road, Erie, PA  16506, rear / west side of the structure, on or about May 27, 2020 and times prior thereto:  Employees were using a ladder to access/egress the roof of the structure.  The ladder was extended 1.5-feet above the upper level landing surface (roof) and was not otherwise secured at its top to a rigid support that will not deflect, and/or equipped with a grasping device to provide assistance employees in mounting and dismounting the ladder.    ABATEMENT CERTIFICATION REQUIRED:  Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
