Located at 388 Faulkner Drive in Lithopolis, Ohio: On or about June 1, 2020, employee were performing framing work from the roof trusses attached to vertical lifelines that were stretched to the sides of the roof and rigged beyond the maximum distance of six foot. The use of the lifelines exposed employees to a fall hazard of approximately ten to twenty foot.
