29 CFR 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall: 29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level. a. Located at 388 Faulkner Drive in Lithopolis, Ohio: On or about June 1, 2020, employee were performing framing work from the roof trusses attached to vertical lifelines that were stretched to the sides of the roof and rigged beyond the maximum distance of six foot. The use of the lifelines exposed employees to a fall hazard of approximately ten to twenty foot.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a. Located at 388 Faulkner Drive in Lithopolis, Ohio: On or about June 1, 2020, the employer did not ensure that employees were trained on the proper use of fall protection equipment while performing framing work from the roof with personal fall arrest systems.
