29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  On and about June 2, 2020, employees were exposed to multiple construction related hazards, including but not limited to, falls, struck by, and electrical, when they had not been trained to recognize and avoid or correct unsafe conditions or to understand and follow the OSHA Standards that applied to their work at 2695 Howard Commons Drive, Howard, WI.

29 CFR 1926.1053(b)(11): Ladders shall not be moved, shifted, or extended while occupied.  On July 2,2020, employees were exposed to falls and struck by hazards while setting up and extending a 36' aluminum extension ladder that did not have the extension rope connected to the fly section, requiring an employee to climb the ladder while extending it, and while the ladder was being held upright by the employer and another employee at 2695 Howard Commons Drive, Howard, WI.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  On and about June 2, 2020, employees were exposed to struck by and fall hazards while setting up and climbing extension ladders when they had not been trained to properly inspect, set up, and use the ladders at 2695 Howard Commons Drive, Howard, WI.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On and about June 2, 2020, two employees were exposed to fall hazards while installing siding on a two story apartment building by walking on the roof from the ladder access to the dormer wall they were working at without personal fall arrest systems and no alternative fall protection measures installed 2695 Howard Commons Drive, Howard, WI. The roof eave height was 18 feet above the ground. The travel distance on the roof  was over 30 feet. The employees had been using the roof area to access their work areas over 3 days without setting up and using fall protection.    Rhyo Siding and Restoration, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1369471, citation number 1, item number 5, and was affirmed as a final order on 8/26/2019, with respect to a workplace located at 364-372 S. Jewel Ct. Palatine, IL 60074.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On and about June 2, 2020, employees working 6 feet or more above a lower surface while installing siding on an apartment building when had not been trained to recognize the fall hazards associated with their work tasks; the correct methods of installation and maintenance for the fall protection systems provided by the employer; the appropriate fall protection system for unique work conditions; and the OSHA Subpart M Fall Protection Standards requirements applicable to the employees work tasks.  Rhyo Siding and Restoration, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1369471, citation number 1, item number 4b, and was affirmed as a final order on 8/26/2019, with respect to a workplace located at 364-372 Jewel Ct. Palatine, Il 60074.
