29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):          (a)   On June 1, 2020, for the employees exposed to fall hazards of approximately 9 feet 2 inches while installing shingles at or near 1213 South Washington Street in Bismarck, North Dakota.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, and the ladder was secured at its top to a rigid support that will not deflect, a grasping device was not provided to assist employees in mounting and dismounting the ladder.         (a)   On June 1, 2020, for the employees exposed to fall hazards of approximately 10 feet where an extension ladder extended only a one and a half foot above the roofline, while performing shingling operations at or near 1213 South Washington Street in Bismarck, North Dakota.
