29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)  490 N. Pin Oak Place - Observed on or about 6/3/2020, employees in the process of roofing activities on the 5 in 12 slope (vertical to horizontal) roof, were exposed to a 25 foot fall hazard, in that, fall protection equipment was not installed or utilized.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a)  490 N. Pin Oak Place - Observed on or about 6/3/2020, Employees were using a ladder that did not have the side rails extended a minimum of 3 feet above the upper landing surface. The employees were exposed to a fall height of about 8 feet.

Pin Oak Place - Observed on or about 6/3/2020, employees were not instructed as to the hazards associated with their work duties including but not limited to, the proper construction, use, and placement of ladders.
