The employer's lockout/tagout program for line breaking did not include procedures including, but not limited to: a) Procedures for ensuring the identity of the hazardous chemical(s) in process piping. b) Procedures for ensuring that employees are trained on the hazards of these chemical(s). c) Procedures for ensuring that the appropriate type(s) personal protective equipment for the hazardous chemical(s) are selected and utilized by employees.
