29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about June 9, 2020, two employees were exposed to an amputation hazard; in that, two employees were sitting on an Alpine Ram Easy rider Truss Roller while the roller was in motion. b. On June 11, 2020, the compliance officer observed one employee exposed to an amputation hazard; in that, the employee was standing on an Alpine Ram Easy rider Truss Roller. IN THE ALTERNATIVE: OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to amputation hazards while operating an Alpine Ram Easy rider roller: a. On or about June 9, 2020, two employees were exposed to an amputation hazard; in that, two employees were sitting on an Alpine Ram Easy rider Truss Roller while the roller was in motion. b. On June 11, 2020, the compliance officer observed one employee exposed to an amputation hazard; in that, the employee was standing on an Alpine Ram Easy rider Truss Roller.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to being struck by polyvinyl chloride pipe fragmentation: a. On or about June 11, 2020 and at times prior, employees working and walking near saw operators, were exposed to the hazard of being struck by flying shards of polyvinyl chloride piping, in that; the compressed air (120 p.s.i) was passed through polyvinyl chloride piping from the air compressor to the branch and air hose connections. b. On or about June 11, 2020 and at times prior, employees working and walking near the Alpine Ram Easy Roller Truss, were exposed to the hazard of being struck by flying shards of polyvinyl chloride piping, in that; the compressed air (120 p.s.i.) was passed through polyvinyl chloride piping from the air compressor to the branch and air hose connections.
