29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms attached to the forks of a forklift: a) Jobsite located at 1020 Christian Street, Philadelphia, PA: The employer did not ensure that employees were protected from falls while working on a scaffold/ platform attached to a forklift at heights greater than 10 feet without the use of a fall protection system; thus exposing employees to fall hazards. Condition was observed on or about 12 JUN 2020.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite located at 1020 Christian Street, Philadelphia, PA.: Employees engaged in framing activities on a residential building were not provided fall protection; thus exposing employees to falls of approximately 20 feet. Condition was observed on or about 12 JUN 2020. The FDR Contractor, Corp., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1448342, citation number 1, item number 3 and was affirmed as a final order on 02-21-2020, with respect to a workplace located at182 Terrace Ave, Lodi, NJ 07644.
