29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) Jobsite located at 2700 East Auburn Street, Philadelphia:  The employer did not have a competent person inspect the scaffolds and scaffold components to inspect for visible defects. The cross brace used to provide stability and structural integrity for the scaffold was not attached to the frame. Scaffolds upright posts and base plates were not installed with the scaffold, and the employer allowed employees to work from the scaffold exposing them to scaffold collapse hazards.  Condition was observed on or about 15 JUN 2020.

Scaffold frame and crossbraces were used as a means of access:  a) Jobsite located at 2700 East Auburn Street, Philadelphia.: The employer did not provide a safe means to access the scaffold for the employees. Employees climbed on the outside of the scaffold to access the work level; thus exposing them to fall hazards.  Condition observed on or about 15 JUN 2020.
