Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made:    (a) On or about June 10, 2020 - Near truck door 37 at transition from old to new building, employees were exposed to struck-by hazards when the metal plate was not flush with the floor on either side of it.
