29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about March 15, 2023, and at times prior thereto, powered industrial truck operators at the facility were not provided with refresher training on a schedule of at least once every three years exposing the employees to struck-by hazards. Southeastern Freight Lines, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii), which was contained in OSHA inspection number1479547, citation number 1, item number2b and was affirmed as a final order on 10/13/2020, with respect to a final abatement date of 10/7/2020 with respect to a workplace located at 2701 Southeastern Circle, BIRMINGHAM, AL 35215.
