29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.  On or about 6/17/20, at the Byron facility, the employer did not assure new tooling used on presses had been X-rayed prior to putting the tooling in service, per the procedure established at the facility. Tooling used on press 8 jammed resulting in a deflagration.

29 CFR 1910.147(c)(6)(i)(B): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about 6/19/20, at the Byron facility, the employer did not ensure a periodic inspection of the Lockout program was conducted to ensure deficiencies were corrected. Deficiencies included:   a - Employees who were servicing and maintaining presses at the site did not know or understand the magnitude of energy for the equipment they were charged with maintaining. Specific procedures did not indicate what the magnitudes of energy were and the employees interviewed indicated they did not know.    b - Procedures did not indicate what piece of equipment the procedure was for and instead addressed all presses despite the fact the presses are on different disconnects.    c - A specific statement of the intended use of the procedure.   d - Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  e - The certification did not identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On 6/19/20, at the Byron facility, the employer did not train press operators who were providing service and maintenance to presses as Authorized employees.
