29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) GRDC Contracting, LLC., at a jobsite located at 9757-9777 Loon Lane, Building DD, Canal Winchester, Ohio 43116: On and/or preceding June 18, 2020, the employer failed to initiate and maintain a safety and health program such as but not limited to a personal protective equipment program, a fall protection program, and a ladder safety program for employees conducting framing work.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) GRDC Contracting, LLC., at a jobsite located at 9757-9777 Loon Lane, Building DD, Canal Winchester, Ohio 43116: On and/or preceding June 18, 2020,the employer did not ensure employees wore safety glasses while utilizing power tools that presented struck-by hazards to the eyes and face. Carpenters were installing roof sheathing while using circular saws and pneumatic nail guns. The employees were not utilizing eye protection, which exposed them to struck-by hazards to the eyes.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a) GRDC Contracting, LLC., at a jobsite located at 9757-9777 Loon Lane, Building DD, Canal Winchester, Ohio 43116: On and/or preceding June 18, 2020, the employer did not ensure employees were adequately protected from falls while engaged in residential construction activities. Employees were sheathing the roof without adequate fall protection, thereby exposed to fall hazards of up to 10 feet 11 inches.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a) GRDC Contracting, LLC., at a jobsite located at 9757-9777 Loon Lane, Building DD, Canal Winchester, Ohio 43116: On and/or preceding June 18, 2020, the employer did not ensure employees used anchorages capable of supporting 5000 pounds while engaged in residential construction activities. Employees were framing and sheathing the roof with lifelines and straps tied around the top of the trusses at the truss nail plate, thereby exposed to fall hazards of 10 feet 11 inches.

Employees were framing and sheathing the roof with ropes and a lifeline that were not adjusted to prevent a free fall to less than 6 feet, thereby exposed to fall hazards of up to 10 feet 11 inches.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  a) GRDC Contracting, LLC., at a jobsite located at 9757-9777 Loon Lane, Building DD, Canal Winchester, Ohio 43116: On and/or preceding June 18, 2020, the employer did not ensure employees used ladders for the purpose in which they were designed. An employee was using a step ladder in the closed position and while leaning against a wall, thereby exposed to falls from the ladder.
