29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a. On or about June 19, 2020, at the above addressed jobsite, employees using a backpack blower did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.502(h)(1)(iii): The safety monitor shall be on the same walking/working surface and within visual sighting distance of the employee being monitored. a. On or about June 19, 2020, at the above addressed jobsite, employees conducting roofing operations were exposed to fall hazards of greater than thirty (30') feet, due to not being in visual sight of the safety monitor, thereby exposing employees to the hazards associated with falls. The Matthews Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.502(h)(1)(iii), which was contained in OSHA inspection number 1260505, citation number 1, item number 1 and was affirmed as a final order on January 11, 2018, with respect to a workplace located at 2101 Scoville Ave., Berwyn, IL.
