29 CFR 1910.95(g)(5)(ii): Where mobile test vans were used to meet the audiometric testing obligation, the employer did not obtain a valid baseline audiogram within 1 year of an employee's first exposure at or above the action level:     (a) In the Slicing/Packoff Line #3, a machine operator was exposed to continuous noise levels at 124.2% of the permissible 8- hour time weighted average (TWA). The equivalent dBA level of 124.2% is approximately 91.5 dBA. Sampling was performed for 420 minutes during one shift. Zero exposure was assumed for the 60 minutes not sampled.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):  (a) In the Slicing/Packoff Line #3, a machine operator was exposed to continuous noise levels at 181.7% of the permissible 8- hour time weighted average (TWA). The equivalent dBA level of 181.7% is approximately 94.3 dBA. Sampling was performed for 461 minutes during one shift. Zero exposure was assumed for the 19 minutes not sampled;   (b) In the Pepperoni stuffing area, a machine operator was exposed to continuous noise levels at 92.04% of the allowable time weighted average (TWA). The equivalent dBA level of 92.04% is approximately 89.3 dBA. Sampling was performed for 465 minutes during one shift. Zero exposure was assumed for 15 minutes not sampled.
