29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a.  On or about June 25, 2020, at the above addressed jobsite, the portable ladder used to access the roof was not able to extend at least three (3) feet above the upper landing surface and was not secured at its top, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a.  On or about June 25, 2020, at the above addressed jobsite, employees performing residential roofing operations, eleven (11) feet nine (9) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.    Jerry's Contracting & Roofing Service, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1263303, citation number 1, item number 1, and was affirmed as a final order on October 26, 2017 with respect to a workplace located at 18306 Black Oak Ave. in Tinley Park, IL.

On or about June 25, 2020, at the above addressed jobsite, the employees conducting residential roofing operations were not provided training on how to minimize fall hazards, thereby exposing employees to the hazards associated with falls.
