29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  (a)  L&L Acoustical, Inc. at 913 Palomino Way in Windsor, CO 80550:  On or before June 25, 2020, the employer did not ensure that all wheels on a single platform baker-style mobile scaffold could be securely locked.  Employee fell from a height of approximately 4 feet when the mobile scaffold wheel rolled into a 5-inch diameter hole in the floor, causing the mobile scaffold to tip over while the employee was standing on the platform of the scaffold.  This condition exposed employee to fall hazard and struck-by hazard from the upended mobile scaffold.
