On or about June 30, 2020, in the frozen food processing area in warehouse 9026 Herring, the exit routes for employees working inside were not identified with a sign reading EXIT, thus exposing employees to hazards, including but limited to, smoke inhalation.  b.  On or about June 30, 2020, in the frozen food processing area in warehouse 9026 Herring, the exit route for forklift operators working inside were not identified with a sign reading EXIT, thus exposing employees to hazards, including but limited to, smoke inhalation.

29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to the operator that has been observed to operate the vehicle in an unsafe manner:  a.  On  or about May 27, 2020, an employee operating a forklift, in the frozen food processing area, was not aware of their surroundings and damage the ceiling tiles when the mast of the forklift was raised too high, thus exposing employees to struck by hazards.  b.  On or about June 7, 2020, an employee, in the frozen food processing area, was operating a forklift in an unsafe manner, in that, a cargo strap was rigged below the forks, using knots in the strap, while lifting a section of the cluster brake and while an employee was standing under the load.

29 CFR 1910.184(c)(2): Slings were shortened with knots or bolts or other makeshift devices:  a.  On or about June 7, 2020, in the frozen food processing area, an employee operating a forklift was using a cargo strap, with knots in it, to shorten the length of the cargo strap. The forklift was lifting a section of the cluster brake and an employee was working directly below the suspended load, thus exposing the employee to laceration, amputation and other hazards.

29 CFR 1910.184(c)(6): Slings were not securely attached to their loads:  a.  On or about June 7, 2020, in the frozen food processing area, an employee operating a forklift was using a cargo strap as a sling to lift and suspend a section of the cluster brake. The cargo strap was rigged below the forks using knots and the strap was wrapped around the cluster brake but not physically attached to the load. An employee was working directly below the suspended load, thus exposing the employee to laceration, amputation and other hazards.

On or about June 30, 2020, employees working in the frozen food processing area were not provided Hazardous Communication training prior to the employees handling and using chemicals including but not limited to Quorum Clear V and Quorum Yellow VP, thus exposing employees to hazards including but limited to skin, eye, and respiratory irritations.
