Atlantic Ave., employees were exposed to caught in between and amputation hazards when the employer did not have clear and specific procedures for shutting down, isolating, blocking or securing a Hughes Equipment food sorting machine while it was being cleaned.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On July 27, 2022 at 9025 N. Atlantic Ave, employees were exposed to caught in between and crushing hazards when an employee was injured while operating a bagging machine. When reaching into the point of operation to reposition a bag the employee got the right thumb caught in the machine and there was not a guard or method to prevent injury.

Atlantic Ave in the warehouse area, employee was exposed to caught in between hazard when a 600 volt breaker panel switch was not made accessible to each employee due to the switch and panel being wrapped in plastic to prevent water from entering it in case of equipment shut down is needed.
