On or about June 28, 2020, located at 1980 Alum Creek Drive, Columbus, Ohio: the employer did not record the injury on the OSHA 300 log of an employee who was injured at worksite located at the vicinity of 668 Indian Mound Road, Columbus, Ohio.  The injury had required the employee to receive medical treatment and hospitalization beyond first aid as a result.
