Builders, LLC, conducting work at 19 Winfield Manor Court, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that employees were using appropriate eye protection such as, but not limited to, safety glasses while conducting construction work with equipment such as, but not limited to, pneumatic nail guns. Employees were exposed to the hazards of eye injury from flying objects.

Builders, LLC, conducting work at 19 Winfield Manor Court, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that employees were protected from fall hazards while working atop the elevated scaffolding platform. The employees were walking and working atop the scaffolding platform, supported by the CAT TL642 extending boom forklift, without any fall protection in place. The employees, working in this condition, were exposed to approximately 11 feet fall hazard from the platform to the ground below.

Builders, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.501(b)(13)), which was contained in OSHA Inspection # 1361781, Citation number 1, Item number 3, and was affirmed as a final order on 03/19/2019, with respect to a workplace located at 460 Cantbury Blvd., Perrysburg, OH 43551.  M.L.S. Builders, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.501(b)(13)), which was contained in OSHA Inspection # 1060031, Citation number 1, Item number 2, and was affirmed as a final order on 05/19/2015, with respect to a workplace located at 276 Tiberon Trace, Wadsworth, OH 44281.
