1910.147(f)(1)(iv): Deenergize all systems and reapply energy control measures i accordance with paragraph (d) of this section to continue the servicing and/or maintenance:  a) Seasoned Cooked Beef Department - On or about July 1, 2020, an employee was exposed to machine hazards associated with rotating blades inside a tube when, after troubleshooting the lack of product flow, the SCB Packoff Supply Pump model 0300 TRAIO, serial number 262987/a/17, was not deenergized in accordance with the employer's written procedure and energy control measures were not reapplied before the employee used a gloved finger in an attempt to unjam the inside of the SCB Packoff Supply Pump, which resulted in the amputation of the tip of the employee's right index finger.
