29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about August 20, 2021, in the roll processing department, the employer failed to provide effective guarding to protect employees working at or near the CP Packaging Roll Stock 104 bag forming machine. The guarding on the machinery did not prevent employees from reaching through the guard and into the zone of danger during the bag forming and packing process.  Employees were, thereby, exposed to amputation hazards.
