29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 7, 2020 at the work site where employees were exposed to a crushed-by hazard as they were working with a HTC press brake machine SN: 582G2432 in which the point of operation wasn't guarded. Protect Controls, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1445960, citation number 1, item number 1 and was affirmed as a final order on 4/24/2020 with respect to a workplace located at 3212 Old Hwy 105 E. Conroe, TX 77301.
