29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  Location: Jobsite  On or about July 14 2020 an employee was exposed to fall hazards of up to 20 feet to the concrete sidewalk below while working on a scaffold system that had unsupported overlap of scaffold planks greater than 18 inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Location: Jobsite  On or about July 14, 2020 an employee was exposed to a fall hazard of up to 30 feet to the concrete sidewalk below while climbing the frame section of a fabricated frame scaffold system.

JOHN WILLIAM MASONRY BRS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926. 20(b)(2) which was contained in OSHA inspection number 1397997, citation number1 , item number 2 and was affirmed as a final order on 9-18-2019, with respect to a workplace located at 121 Portland Street Boston MA.
