29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. a) New townhome construction located on Lots #18-20, Second level: On or about 07/09/2020, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that eye protection was not provided to employees engaged in roof truss installation. b) New townhome construction located on Lots #18-20, Second level: On or about 07/09/2020, the employer exposed employees to overhead struck-by hazards while installing roof trusses, in that head protection was not provided while working under crane hoisted load of roof trusses weighing up to 500 lbs.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) New townhome construction located on Lots #18-20, Back side of the structure: On or about 07/09/2020, the employer failed to inspect all electrical equipment and electrical cords prior to the start of the shift, in that extension cords used to power circular saws were damaged and exposing internal wires and not repaired or removed before the start of work.
