29 CFR 1926.651(j)(2): Employees shall be protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection shall be provided by placing and keeping such materials or equipment at least two feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On July 9, 2020, at a worksite located at 809 5th Avenue, in Antigo, Wisconsin, employees were exposed to struck-by hazards while working in an excavation where the spoil was not held at least two feet away from the south side of the excavation or retained by devices sufficient to prevent materials from falling into the excavation.

29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were four feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On July 9, 2020, at a worksite located at 809 5th Avenue, in Antigo, Wisconsin, employees were working in an excavation approximately seven-feet-deep when the ladder used for egress had been removed from the trench box and the slope at the west end was too steep for employees to climb without using their hands.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On July 9, 2020, at a worksite located at 809 5th Avenue, in Antigo, Wisconsin, an employee was exposed to engulfment hazards in a seven-foot-deep trench when the employee was working outside, and alongside, of the six-foot-high, 24-foot-long trench box. A-1 Excavating, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1078205, citation number 1, item number 1, and was affirmed as a final order on 2/5/2016, with respect to a workplace located on Valley Ridge Rd. Middleton, WI 53562.
