29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  (a) Excavation site located at 2nd Street, Hayward, WI; On or about June 27, 2024, a means of egress was not readily accessible for three employees while they worked in an excavation that was approximately 50 feet long and ranged in depth from 8-feet, 5-inches to 8-feet, 3-inches at their observed work areas.  Sidewalls of the excavation were too steep to be considered as walkable earthen ramps and both ladders in the excavation were located near each other at the opposite end of the excavation, which was approximately 31 feet away from the nearest worker in the excavation.  The A-1 Excavating, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1482923, citation number 1, item number 2 and was affirmed as a final order on February 20, 2021, with respect to a workplace located at 809 5th Ave., Antigo, WI 54409.
