29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems: (a) 3340 Liberty Avenue, Pittsburgh, PA 15201, On or about July 14th 2020 employees were not protected from fall hazards of approximately 35 feet while installing a new roof on a structure that was measured at a 10/12 pitch. Employees were installing a slate roof and no fall protection was in use. i.e. harnesses, guard rails, etc.

The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel. The employees were exposed to a fall hazard of approximately 30 feet to the ground below.
