29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: A.) Lake City Industries, Inc., 1420 Cherry Street, Lake City, PA 16423, sanding station, on or about July 17, 2020 and times prior thereto: A Bright Way MP6DG relocatable power tap was plugged into a an E-Zee Lock device and suspended vertically above the work area. There were no strain relief devices installed where the flexible cord entered the power tap's housing. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: A.) Lake City Industries, Inc., 1420 Cherry Street, Lake City, PA 16423, aluminum deburring/buffing station, on or about July 17, 2020 and times prior thereto: Employees were utilizing an motor and shaft system that was equipped with a buffing wheel to debur / buff aluminum parts. The exposed rotating shaft between the motor and the buffing wheel was not guarded. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The rear plastic motor housing was cracked. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
