Audiometric testing was last provided January 21, 2015.     b) Lake City Industries, Inc.- On or about August 19, 2020, a machine operator was exposed to a mixed noise level of 87.9 dBA or an equivalent dose of 74.50%, as an 8-hour TWA, while operating a punch press.  This level was based on 466 minutes of sampling; zero exposure was considered for the unsampled time. Audiometric testing was last provided January 21, 2013.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

On or about 7/29/20 and 8/19/20 a laborer conducting deburring was observed wearing a N95 respirator improperly, in that top and bottom straps of the employees respirator were behind the employees neck, and were not properly placed on the back of the employee's head.   b) Lake City Industries, Inc. - The employer did not ensure that users of N95 respirators could demonstrate knowledge of how to put on and use N95 respirators.  On or about 7/29/20 and 8/19/20 a laborer utilizing a hand grinder was observed wearing a N95 respirator improperly, in that the bottom strap of the employees respirator was not in use.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
