29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    At jobsite 368 S 143rd St E Wichita KS,  the employer did not obtain written approval from the manufacturer to use an attachment on the Terex Telehandler, model number TH636C Turbo, and did not ensure that markings for the capacity, operation and maintenance were changed accordingly. This condition exposes employees to struck-by and crushing hazards.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At jobsite 368 S 143rd St E Wichita KS, employees were exposed to fall hazards.  Employees engaged in residential framing activities, using a Werner 16 foot metal extension ladder, model #D1216-2, were exposed to fall hazards of approximately 10 feet from the eve of the building to the ground below. Employees were exposed in that the ladder was not secured to the building and did not extend at least 3 feet above the eave.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    At jobsite 368 S 143rd St E Wichita KS, employees engaged in framing activities were exposed to fall hazards of approximately 10 feet from the working surface to the ground in that fall protection was not utilized.    Titan Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1343416, citation number 1, item number 1 and was affirmed as a final order on 09/19/2018, with respect to a workplace located at 6322 W. Venice St. Wichita, KS 67205.
