WM Recycle America, L.L.C. directed employees of Elite Labor Services, Ltd. to enter the OCCScreen machine through a small access hatch to dislodge debris collected within the spindles or discs. The screening machine utilized electricity to rotate the spindles. The workers moved within the screening machine upon the unstable surface of the spindles and discs.

29 CFR  1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  (a)  WM Recycle America, L.L.C., at 5395 Franklin Street, Denver, CO:  On and preceding 7/20/2020, WM Recycle America, L.L.C. did not ensure that the energy control procedures clearly and specifically outlined the steps for shutting down and isolating the CP Manufacturing ONP screening machine.  The written procedure did not include instruction to lower the hydraulic deck to its lowest level prior to de-energization and isolation of electrical power to the ONP screening machine.   This condition potentially exposed employees of WM Recycle America, L.L.C. and employees of Elite Labor Services, Ltd. to an uncontrolled energy hazard.
