The mobile ladder was used by employees to obtain items from the shelving units and was not stable nor did the braking mechanism work exposing employees to potential injury from tip over hazards.

29 CFR 1910.23(e)(1)(vii): (vii) Mobile ladder stands and platforms that have wheels or casters are equipped with a system to impede horizontal movement when an employee is on the stand or platform: (a) Facility, 2965 Pittsburgh Road, Perryopolis, PA: On or about July 22, 2020 a mobile ladder stand did not impede horizontal movement when employees were on the platform. The braking system was not operable and the ladder would move and not stay in place when used by employees to obtain stock from shelving units.

Employees cut various sizes of materials as small as 2" x 2" and are exposed to the rotating blade. (b) Facility, Ball Transfer, 2965 Pittsburgh Road, Perryopolis, PA: On or about July 22, 2020 no guard was in place on the lathe used in the tool & die department. An employee uses the lathe to polish tooling and is exposed to the rotating chuck and flying debris.

An employee works at the table processing tubes to make neon lights and is exposed to potential electrical hazards.
