29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) In the Ball Transfer Stamping Area, on or about August 17, 2023 - The employer failed to administer a hearing conservation program including an audiometric testing program when a Production Worker's noise exposure exceeded a dose of fifty percent while operating presses.  Image Time Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.95(c)(1), which was contained in OSHA inspection number 1484766, citation number 1, item number 1a and was affirmed as a final order on March 26, 2021, with respect to a workplace located at 2965 Pittsburgh Road, Perryopolis, PA 15473.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a) In the Ball Transfer Stamping Area, on or about August 17, 2023 - The employer failed to make audiometric testing available to a Production Worker who was exposed to noise that exceeded an 8-hour time-weighted average of 85 decibels while operating presses.  Image Time Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.95(g)(1), which was contained in OSHA inspection number 1484766, citation number 1, item number 1c and was affirmed as a final order on March 26, 2021, with respect to a workplace located at 2965 Pittsburgh Road, Perryopolis, PA 15473.

29 CFR  1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a) In the Powder Coating Area, on or about August 17, 2023 - The employer did not implement elements of a written program necessary to ensure that a Powder Coater voluntarily wearing an elastomeric half mask air purifying respirator while powder coating brackets was medically able to use that respirator.
