On or about July 24, 2020 at the jobsite located at 7123 Sauk Trail Rd Sheboygan, WI, a fork-lift was used to support a scaffold platform that was not entirely attached to the forks of the fork-lift.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about July 24, 2020 at the jobsite located at 7123 Sauk Trail Rd Sheboygan, WI, an employee engaged in roofing activities on a low-slope roof with an unprotected side 17' above surface level was not protected from falling by a conventional form of fall protection.

On or about July 24, 2020 at the jobsite located at 7123 Sauk Trail Rd Sheboygan, WI, an employee on a 28' fiberglass extension ladder shifted the ladder to the right while he was using a pneumatic nailgun to secure plywood sheets in place.
